چکیده
2.1. In patients with acute DVT of the leg treated with vitamin K antagonist (VKA) therapy, we Background: This article addresses the treatment of VTE disease. Methods: We generated strong (Grade 1) and weak (Grade 2) recommendations based on highquality (Grade A), moderate-quality (Grade B), and low-quality (Grade C) evidence. Results: For acute DVT or pulmonary embolism (PE), we recommend initial parenteral anticoagulant therapy (Grade 1B) or anticoagulation with rivaroxaban. We suggest low-molecular-weight heparin (LMWH) or fondaparinux over IV unfractionated heparin (Grade 2C) or subcutaneous unfractionated heparin (Grade 2B). We suggest thrombolytic therapy for PE with hypotension (Grade 2C). For proximal DVT or PE, we recommend treatment of 3 months over shorter periods (Grade 1B). For a fi rst proximal DVT or PE that is provoked by surgery or by a nonsurgical transient risk factor, we recommend 3 months of therapy (Grade 1B; Grade 2B if provoked by a nonsurgical risk factor and low or moderate bleeding risk); that is unprovoked, we suggest extended therapy if bleeding risk is low or moderate (Grade 2B) and recommend 3 months of therapy if bleeding risk is high (Grade 1B); and that is associated with active cancer, we recommend extended therapy (Grade 1B; Grade 2B if high bleeding risk) and suggest LMWH over vitamin K antagonists (Grade 2B). We suggest vitamin K antagonists or LMWH over dabigatran or rivaroxaban (Grade 2B). We suggest compression stockings to prevent the postthrombotic syndrome (Grade 2B). For extensive superfi cial vein thrombosis, we suggest prophylactic-dose fondaparinux or LMWH over no anticoagulation (Grade 2B), and suggest fondaparinux over LMWH (Grade 2C). Conclusion: Strong recommendations apply to most patients, whereas weak recommendations are sensitive to differences among patients, including their preferences. CHEST 2012; 141(2)(Suppl):e419S–e494S

Antithrombotic therapy in children with venous thromboembolism.
Antithrombotic therapy has recently become more frequent for the treatment of venous thromboembolism (VTE) in the paediatric population. This can be explained by the increased awareness of morbidities and mortalities of VTE in children, as well as the improved survival rate of children with various kinds of serious illnesses. Considering the large number of years a child is expected to survive,...

Heart failure (HF) represents a major and growing public health problem because of its prevalence, incidence, morbidity, mortality, and economic costs. The prevalence of HF is 2% to 3% of the general population.1 Five million Americans are affected, with 550 000 cases diagnosed each year.2 The mortality rate from severe HF remains 60% within 5 years of diagnosis, and 50% of hospitalized patient...

INTRODUCTION The aim of this study was to determine the prevalence of venous thromboembolism (VTE) after elective thoracic surgery in patients receiving antithrombotic prophylaxis, and to evaluate the risk of pulmonary embolism (PE) after lung resection. PATIENTS AND METHOD A descriptive, cross-sectional, retrospective study was designed. A total of 6004 patients were included. All patients u...
متن کاملAntithrombotic effects of hydroxychloroquine in a pregnant patient with Antiphospholipid syndrome and recurrent venous thromboembolism.
A pregnant woman with antiphospholipid syndrome presented with repeated venous thromboembolism (VTE) in the first and second trimesters of pregnancy despite receiving combination therapy with low-molecular-weight heparin and aspirin. The addition of hydroxychloroquine prevented further VTE recurrence, thus demonstrating its potential antithrombotic effects.
متن کاملTreatment of venous thromboembolism: challenging the unfractionated heparin standard.
Venous thromboembolism (VTE) is a major public health problem leading to high morbidity and mortality in the United States. Since more then 50% of patients with VTE may have asymptomatic disease, the start of appropriate therapy often is delayed. Traditionally, intravenous unfractionated heparin (UFH) has been used to manage the acute phase of VTE. Although an effective agent, numerous limitati...
